in the United States with any visa other than an F-1 or J-1 or
J-2, attach a copy of your visa and both sides of your I-94.

of funding is United States or home-country government or an international
organization. (J-1 dependents will be issued J-2 visas.)

(for J-1 Exchange Visitor/Student visa) I am on a university-to-university
exchange program.
